Rates and Insurance

Call for Individual Face to Face rates each session is  45-50 minutes

 (Tele Health ) is available via phone, or video conferencing 

Please call for rates. Client must reside in the state of California and have a current California I.d that verifies their residence. A phone consult is needed before booking an appointment.

Intake forms and valid I.d such as a California driver's license, or California's state issued I.d must be received prior to scheduling If you are new to the state of California proof of residence will need to be faxed. Payment is due prior or at the beginning of your session. In order to maximize your phone session a payment should be made prior to the session scheduled if you're paying out of pocket.
A 48 hour cancellation policy applies to all session scheduled appointments. Same day cancellations will result in the session fee being charged to your credit card account. 

 I'm currently an in network provider for Anthem Blue Cross/ LA Care, Optum, Cigna, MHN  and Single case agreements for Blue Cross Blue Shield. I'm not a medical or HMO provider these plans require that you go through your network to find a medical provider that is accepting new patients.
 

Please check your coverage carefully by asking the following questions: 

  • Do I have mental health insurance benefits?
  • Is the provider I want to see in network
  • What is my deductible and has it been met?
  • What is my co-pay for mental health services
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician

Payments accepted
Cash,visa and mastercard are accepted for payment. A credit card processing fee  will apply for each transaction. Checks are not accepted

Co-pays are set by your health care plan. These fees are non negotiable and must be paid on the date of service.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not  given notification at least 48 hours in advance, you will be required to pay the full cost of the session. If you are utilizing insurance to cover your sessions, you will be required to pay the full cost of the session if you have not notified us  48 hours in advance.
